Main Deck Structure: 11/15 - You really don't need 3 of Tin Goldfish. He was made for Xyz Summons, and since you only have 1 Xyz he is unnecessary, even if he is in there to Special Summon Shadow Mist to search a Change card. Form Change is not a very good choice either since it is too specific. You are better off with just 1 of it (if you HAVE to have it) and swapping the other for another Mask Change Second.

Main Deck Synergy: 10/10 - No issues on synergy whatsoever. All comes together nicely.

Side Deck: 3/10 - The only cards that belong there are the Imprisoning Mirrors and perhaps Torrential. Everything else is really Main Deck stuff. Had stuff like Pot of Duality, Number 101, Exciton Knight, etc. Not very effective in the current format.

Extra Deck: 7/10 - Kamikaze is a bad choice, since the only possible way you can summon him is with Form Change. You will also NEVER need more then 1 Chaos since he isn't that easy to summon. Swap out Kamikaze and 1 Chaos for the Xyz you currently have in your Side.

Use of Cards: 7/10 - Played them like he knew what he was doing, but made several unnecessary moves, such as using Miracle Fusion when he didn't need to, and when I used Mirror Force he had just about nothing to come back at me with. Try not to go all out by doing everything you possibly can in one turn unless you have to (or you have a guaranteed OTK).

Concentration: 10/10

Control of the Duel: 5/10 - You had good control in the first round. Second round it went back and forth. Third round was majorly in my control.

Siding Skills: 0/5 - Taking out Mirror Force for Super Poly was a very bad call. You were better off not siding at all.

Ruling Test Score: 0/10
